Surgical and non-surgical options for upper eyelid lifting

The surgical and non-surgical methods compared, with help in choosing the procedure that suits you.

Dermacare focuses on two effective methods for upper eyelid lifting: the classic surgical upper eyelid lift and the modern, non-surgical treatment with the CO2 laser. Both procedures have their specific advantages and are tailored individually to the patient's needs at our practice.

The classic surgical upper eyelid lift, also known as blepharoplasty, is a proven procedure for permanently removing excess skin and fatty tissue from the upper eyelids. The operation is, as a rule, performed under local anaesthesia and takes about one to two hours. A precise incision is made along the natural eyelid crease in order to remove excess skin and, if necessary, to tighten the underlying tissue. After healing, the scar is barely visible, as it lies in the natural eyelid crease.

Advantages of the surgical upper eyelid lift:

  • Long-term results: The surgical removal of excess skin leads to long-lasting results, which often last for many years.
  • Comprehensive tightening: For pronounced hooded eyelids or larger amounts of excess skin, the operation offers the most effective and comprehensive solution.
  • Improved field of vision: In cases where hooded eyelids restrict the field of vision, blepharoplasty can bring not only aesthetic but also functional benefits.

Disadvantages of the surgical upper eyelid lift:

  • Longer downtime: After the operation, a recovery period of several weeks is necessary, during which swelling, bruising and scar care are required.
  • Invasive procedure: As it is an operation, there are the typical surgical risks such as infections, bleeding or intolerances.

The upper eyelid lift with the CO2 laser is a gentle, non-invasive alternative to surgical blepharoplasty. This procedure uses the thermal energy of the laser to tighten the skin precisely and to stimulate collagen production, without incisions or stitches. The laser vaporises excess tissue and smooths the skin, creating a radiant, more youthful appearance. This method is particularly suitable for patients with moderate hooded eyelids or slight skin laxity.

Advantages of CO2 laser treatment:

  • Minimal downtime: Compared with surgery, patients can return to their daily routine more quickly after the laser treatment, as healing generally takes place within a few days.
  • Gentle method: As no incisions are required, the risks of complications such as scarring, infections or bruising are significantly lower.
  • Promotion of collagen formation: The CO2 laser stimulates the skin to form new collagen, which improves and tightens the skin structure in the long term.

Disadvantages of CO2 laser treatment:

  • Several sessions necessary: While surgical tightening is completed in a single session, several treatments are often required for optimal results with the CO2 laser.
  • Not suitable for everyone: For pronounced hooded eyelids or excess skin tissue, laser treatment reaches its limits and may not be able to achieve satisfactory results.

The choice between surgical tightening and laser treatment depends on various factors, such as the degree of skin laxity and your individual goals. While surgical blepharoplasty delivers excellent results above all for pronounced hooded eyelids, CO2 laser treatment offers a gentle, non-invasive alternative for moderate cases and for improving skin structure.

How does the upper eyelid lift work?

The operation is carried out as an outpatient procedure under local anaesthesia. With the laser variant there are no incisions or stitches, and healing usually takes only a few days.

  1. Initial consultation

    approx. 30 min

In a detailed preliminary discussion, the doctor clarifies your wishes, risks and side effects and decides with you between surgery and the CO2 laser. Blood-thinning medication should be stopped two weeks before an operation.

  2. The treatment

    approx. 1–2 hrs

The operation takes place as an outpatient procedure under local anaesthesia, and the procedure itself takes 30 to 40 minutes. The incision lies in the natural eyelid crease, so the scar is barely visible.

  3. Aftercare

    5–7 days

Cool the wound, sleep with your upper body raised and avoid physical exertion for 14 days. After 5 to 7 days, the stitches are removed at the follow-up appointment in about 15 minutes.

  4. Result

    after 1–2 weeks

As a rule, you are presentable again after one to two weeks. The result of the operation lasts about 8 to 12 years, and with the laser variant several sessions are often necessary.

Frequently asked questions about upper eyelid lifting

After a detailed preliminary consultation, which takes about half an hour, the procedure is performed on an outpatient basis under local anesthesia. Including preparation, you can expect the treatment to take about an hour; the actual procedure usually takes 30 to 40 minutes.

After 5-7 days the stitches are removed at a follow-up appointment, which takes 15 minutes.

The procedure is carried out on an outpatient basis under local anaesthetic on the premises of the Dermacare institute.

As a rule, an upper eyelid lift achieves aesthetic results that are permanently effective. You will look younger and fresher and – if drooping eyelids previously restricted your vision – you will also be able to see better.

The result of a professionally performed eyelid lift lasts about 8-12 years.

Immediately after the operation it is advisable to cool the wound, as this can reduce swelling. Cooling eye masks are available in shops for this purpose. Sleeping with your upper body raised also helps to counteract swelling. Physical exertion should be avoided for at least 14 days after the procedure. When showering and washing your hair, the wound should be kept as dry as possible; eye make-up should be avoided, as should direct sunlight.

In the case of a medical indication, namely a restricted field of vision, this must be established by an ophthalmologist. With a certificate, you can apply to your health insurer for reimbursement of the costs. As a rule, health insurers reimburse up to 400 euros.

After the procedure, patients should rest for at least three days. For 2–4 weeks – depending on the type of sport – you should avoid sport, physical exertion, direct sun and saunas. Contact lenses must not be worn for at least 3 weeks.

If the healing process is normal, the upper eyelid can be made up again 10-14 days after the procedure.

During the consultation, the treating doctor will inform you individually about what you need to observe before and after the operation and will inform you comprehensively about possible risks and side effects. In general, anticoagulant or blood-thinning medication should be stopped two weeks before the surgical procedure. If taking it is medically necessary, this should be discussed with the treating doctor. Please note that many non-prescription medicines, such as cold and flu remedies and painkillers, also contain acetylsalicylic acid or acetylsalicylate and thin the blood. Vitamins C and E must also not be taken before the procedure. Before the procedure, you must have medical clearance for surgery under local anaesthetic. For aftercare, we recommend obtaining a cooling mask or cool packs from a pharmacy or drugstore.

The surgery is performed with the eye closed on the surface of the eyelid so that the eye is not affected. The experienced specialist takes care to avoid injuries. The risk is very small.

As a rule, it is possible to resume everyday life without restrictions after one week, in rare cases after two.

Eyelid surgery is one of the most frequently performed cosmetic procedures and is considered low-risk, but like any surgical procedure it carries the risk of possible complications. It is therefore all the more important to have the treatment carried out by an experienced specialist doctor who knows the risks and side effects and can respond should any occur. Your treating doctor will inform you thoroughly during the mandatory preliminary consultation. It is normal for bruising and swelling to occur after an upper eyelid lift. How quickly these disappear depends on individual wound healing and varies widely. Temporary altered sensation at the edge of the eyelid and a feeling of dryness in the eyes are also harmless after-effects of the operation. Postoperative complications such as bleeding, infection, wound healing problems or troublesome scarring are very rare; in such cases the treating doctor will decide on the further course of action. Nerve damage in the adjacent area of the face occurs very rarely. If you experience any symptoms or have any concerns, please contact us immediately.

When the upper eyelid is corrected, the incision is placed so that the scar is hidden in the natural eyelid crease. Two weeks after the procedure you may wear make-up on the eyelid again. After a few months the scar usually fades, but like any scar it should be protected from direct sunlight. We recommend using sun protection with a factor of 30 or higher for 1 year.

Direct sunlight should be avoided for at least one week. Like any scar, the scar should always be protected from direct sunlight. We recommend using sunscreen with a factor of 30 or higher.

The follow-up check after upper eyelid lifts is carried out by the treating specialist doctor.
